One Year, $100,000 in Capacity Building Funds
The HRSA 25-037 Rural Health Network Development Planning Program aims to increase the capacity of healthcare networks that aim to improve access to care and quality of care in HRSA-designated rural areas. This program supports one year of planning and brings together key parts of a rural health care delivery system, particularly those entities that may not have collaborated in the past, to establish and/or improve local capacity in order to strengthen rural community health interventions, and enhance care coordination.

1. HRSA Rural Health Grants Eligibility Analyzer
Are your offices, consoritum members, or intended service areas located in geographic areas eligible for HRSA Rural Health grants? Enter an address or choose a state and county to check for eligibility status.

4. State Offices of Rural Health (SORH) Contacts

A SORH's purpose is to help individual rural communities build healthcare delivery systems. SORHs are often based at state health departments, but in a few states they are independent organizations or be based in universities.

Applicants must notify their SORH of their intent to apply and obtain a response, which will be included as Attachment 1 in the grant application.

SORH's can be excellent resources of data, ideas, and potential partnerships. They may be able to connect you with other healthcare entities in or around your targeted rural area, and help you understand the statewide context of rural health programs in your state.

5. Partnership Assessment Tool
The Partnership Assessment Tool for Health (PATH), designed for CBOs and health care organizations in existing partnerships, provides a template to: (1) understand progress toward benchmarks characteristic of effective partnerships; (2) identify areas for further development; and (3) guide strategic conversations. The objective of the tool is to help partnering organizations work together more effectively to maximize the impact of the partnership.
